HIPAA notice of privacy practices

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Belle Medical Group LLC (“we,” “our,” or “us”) is committed to protecting your medical information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

We are required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of this Notice as long as it is in effect.

2. How We May Use and Disclose Your Health Information

We may use and disclose your PHI in several ways. For treatment, we may use your information to provide, coordinate, or manage your healthcare and related services. For payment, we may use it to obtain reimbursement for services provided to you. For healthcare operations, your information may be used for administrative, educational, quality assurance, and business management purposes. In matters of public health and safety, we may use your PHI to prevent or control disease, report abuse or neglect, or prevent serious threats to health or safety. In certain circumstances, we may disclose information to law enforcement in response to a court order, subpoena, or other lawful process.

You may also ask us to amend your records if you believe they are inaccurate. You may also request a copy of this Notice in either paper or digital form at any time. Finally, you have the right to file a complaint if you believe your privacy rights have been violated.

4. Changes to This Notice

We reserve the right to change this Notice at any time. Any updates will apply to all PHI we maintain and will be posted on our website with the new effective date.

5. Complaints
